ABSTRAK Accreditation is an external quality assurance system that aims to determine the eligibility of study programs and tertiary institutions based on criteria that refer to the National Higher Education Standards carried out by BAN-PT, while study program accreditation is carried out by the Independent Accreditation Institute (LAM). In conducting study program accreditation, LAM-INFOKOM requires instruments that contain information about the performance of study programs at a tertiary institution, one of which is the Study Program Performance Report instrument. However, in collecting performance data, the Informatics Engineering study program at Bhayangkara University, Surabaya, is still experiencing difficulties because the data has not been properly managed. The absence of a unified data storage facility overwhelmed the form preparation team because they had to collect and recap data collected from various sources. This condition is the problem raised in this study.The results of this study are in the form of a study program performance information system with reference to the study program accreditation form LKPS instrument that has been established by the relevant institution, namely LAM-INFOKOM.
